Dipeptidyl peptidase-4 (DPP-4) is an enzyme present on the surface of many cell types and circulating in blood, where it rapidly breaks down a class of gut-derived hormones called incretins, principally glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). These hormones are released after meals and help the pancreas release insulin in a glucose-dependent manner while suppressing glucagon, a hormone that raises blood sugar. By inhibiting DPP-4, drugs in this class prolong the activity of endogenous incretins, leading to improved insulin secretion and reduced glucagon output after eating, without directly stimulating insulin release independent of glucose levels. This mechanism is central to the management of type 2 diabetes, where impaired incretin signaling and insulin resistance contribute to elevated blood glucose. Because DPP-4 inhibition works through amplifying a natural, glucose-sensitive pathway rather than forcing insulin release, it is generally associated with a lower risk of hypoglycemia compared to some older glucose-lowering approaches. The enzyme's broad distribution across immune, renal, and endothelial tissue also underlies ongoing interest in effects beyond glycemic control, though the primary rationale for this drug class remains restoration of postprandial incretin activity in diabetes care.
