The glucocorticoid receptor (GR) is a nuclear hormone receptor that, once bound by cortisol or a synthetic corticosteroid, moves into the cell nucleus and regulates the transcription of many genes. Its natural role is to coordinate the body's response to stress and to restrain inflammation and immune activity. Because of this, GR agonists (corticosteroids) are among the most widely used anti-inflammatory and immunosuppressive medicines, applied topically for skin disease, inhaled for asthma and chronic obstructive pulmonary disease, and given systemically for a broad range of inflammatory and autoimmune conditions. The same receptor, however, also governs glucose metabolism, bone turnover, and fat distribution, so sustained activation can produce metabolic and structural side effects. This has motivated a complementary strategy: selective glucocorticoid receptor modulators and antagonists, which block or fine-tune GR signaling rather than activate it broadly. These agents are relevant in conditions of glucocorticoid excess, such as endogenous cortisol overproduction, or when clinicians wish to counteract corticosteroid-driven metabolic effects while preserving other treatment goals. Together, agonists and modulators illustrate how one receptor can be therapeutically exploited from opposite directions depending on the clinical need.
