This mechanism combines two distinct drug targets, the beta-2 adrenergic receptor and the glucocorticoid receptor, delivered together by inhalation to treat obstructive airway diseases such as asthma and chronic obstructive pulmonary disease (COPD). The beta-2 adrenergic receptor is a cell-surface receptor on airway smooth muscle; a long-acting beta-agonist (LABA) activates it to relax that muscle, widening the airway and easing airflow. The glucocorticoid receptor is an intracellular receptor found in immune and epithelial cells lining the airway; an inhaled corticosteroid (ICS) activates it to dampen the production of inflammatory signaling molecules, reducing swelling, mucus secretion, and immune cell recruitment within the airway wall. These two problems, muscular narrowing and chronic inflammation, often occur together and reinforce one another, so treating only one leaves the disease process active. A bronchodilator alone can improve symptoms temporarily while inflammation continues unaddressed, whereas pairing it with an anti-inflammatory agent targets both mechanisms simultaneously. This combined approach is a foundational strategy across persistent asthma and stable COPD management, and inhaled delivery concentrates the effect in the lung while limiting exposure of the rest of the body to either drug.
