The beta-2 adrenergic receptor is a cell-surface receptor found abundantly on the smooth muscle that surrounds airways in the lungs. It belongs to the G protein-coupled receptor family, and its natural ligands are adrenaline and, to a lesser extent, noradrenaline. When activated, this receptor triggers relaxation of airway smooth muscle, which widens the airway lumen and eases airflow. This effect is exploited pharmacologically through beta-2 agonists, small molecules that mimic the natural ligand at this receptor. Short-acting agonists produce rapid but relatively brief bronchodilation, useful for quickly relieving acute breathlessness, while long-acting agonists provide more sustained relaxation for ongoing symptom control. Because the receptor also exists in other tissues, agonism is not perfectly confined to the lungs, which underlies some of the class's known side effects. This mechanism is central to managing obstructive airway diseases such as asthma and chronic obstructive pulmonary disease (COPD), where narrowed airways from smooth-muscle contraction, inflammation, and mucus contribute to breathing difficulty. Beta-2 agonists are frequently combined with inhaled corticosteroids, which address underlying airway inflammation, or with agents that block a different receptor pathway (muscarinic antagonists), reflecting the complementary, multi-pathway nature of airway disease management.
