Thymic stromal lymphopoietin (TSLP) is a cytokine produced by epithelial cells lining the airways, skin, and gut. It belongs to a class of signaling molecules called "alarmins," released when epithelial barriers are irritated or damaged by allergens, viruses, pollutants, or other environmental triggers. TSLP acts near the top of a cascade known as Type 2 inflammation, an immune response pattern central to allergic and eosinophilic diseases such as asthma, atopic dermatitis, and chronic rhinosinusitis. By engaging its receptor complex on immune cells, TSLP promotes dendritic cell maturation and activates innate lymphoid cells and T helper cells that release downstream mediators (interleukin-4, interleukin-5, interleukin-13) responsible for airway inflammation, mucus overproduction, and eosinophil recruitment. Because TSLP sits upstream of these multiple downstream cytokines, a monoclonal antibody that neutralizes TSLP prevents it from engaging its receptor, dampening the initiation of the inflammatory cascade rather than blocking one single downstream mediator. This upstream positioning is the rationale for targeting TSLP in conditions where inflammation is heterogeneous or not driven by a single cytokine, potentially offering broader effects across both classic allergic (eosinophilic) and non-allergic inflammatory presentations of respiratory disease.
