Interleukin-5 (IL-5) is a signaling protein, or cytokine, that controls the production, maturation, and survival of eosinophils, a type of white blood cell involved in immune defense against parasites and in allergic inflammation. In several chronic diseases, eosinophils accumulate in tissues such as the airways, skin, or gastrointestinal lining and contribute to inflammation, tissue damage, and symptoms like wheezing or swelling. This pattern of disease is often called eosinophilic or type 2 (T2) inflammation. Ligand-neutralizing antibodies against IL-5 are humanized monoclonal antibodies engineered to bind circulating IL-5 directly, preventing it from attaching to its receptor, IL-5 receptor alpha (IL-5Rα), on the surface of eosinophils and their precursors. By intercepting the cytokine itself rather than blocking the receptor, this approach reduces the signal that eosinophils depend on for maturation and prolonged survival, leading to lower eosinophil counts in blood and tissue. This strategy broadly matters in conditions where eosinophil-driven inflammation is a central disease driver, including certain severe asthma phenotypes and other eosinophilic disorders, offering a targeted way to dampen this specific inflammatory pathway without broadly suppressing the immune system.
