PDE3 and PDE4 are enzymes belonging to the phosphodiesterase family, which breaks down cyclic AMP (cAMP), a signaling molecule that relays messages inside cells once a receptor at the cell surface has been activated. Higher intracellular cAMP levels generally relax smooth muscle and dampen inflammatory cell activity. PDE3 is prominent in airway smooth muscle, where its inhibition promotes bronchodilation, while PDE4 is prominent in immune and inflammatory cells, where its inhibition reduces the release of inflammatory mediators. A dual PDE3/PDE4 inhibitor is designed to combine these two effects in a single molecule: opening the airways directly, in the manner of a bronchodilator, while also tempering the underlying airway inflammation, in the manner of an anti-inflammatory agent. This dual action is particularly relevant in chronic obstructive airway diseases such as COPD, where both persistent airflow limitation and low-grade inflammation contribute to symptoms. Selectively inhibiting only one of the two enzymes typically yields either bronchodilation without meaningful anti-inflammatory benefit, or the reverse; combining both activities in one agent aims to address both dimensions of disease with a single, inhaled or orally administered small molecule.
