GPRC5D is an orphan G protein-coupled receptor, meaning it belongs to a large family of cell-surface signaling proteins but its natural activating molecule remains unidentified. What makes it useful in medicine is not its signaling function but its pattern of expression: it is displayed at high density on malignant plasma cells in multiple myeloma, a cancer of antibody-producing immune cells, while being largely absent from most other tissues in the body. This selective expression allows it to serve as a marker that immune-based therapies can use to distinguish tumor cells from healthy ones. Modulating this target does not aim to block a signaling pathway, as with many drugs, but rather to flag GPRC5D-bearing cells for destruction by engineered immune cells or targeted biologic agents. This approach is particularly relevant for multiple myeloma patients whose disease has stopped responding to therapies directed at other plasma cell markers, since GPRC5D offers an independent point of attack.
