CD40 ligand (CD40L, also called CD154) is a protein expressed transiently on activated T lymphocytes that binds CD40 on B cells, dendritic cells, and other antigen-presenting cells. This CD40-CD40L interaction delivers a costimulatory signal required for full immune activation: it licenses B cells to proliferate, undergo antibody class-switching, and mature into long-lived plasma cells and memory cells, and it promotes dendritic cell maturation and cytokine production. Because this pathway is central to converting antigen recognition into a durable, coordinated immune response, blocking it can dampen pathogenic autoimmunity without broadly destroying immune cells, as many autoimmune and inflammatory diseases depend on sustained T-cell/B-cell crosstalk to generate autoantibodies or self-reactive effector cells. Antibodies directed against CD40L interrupt this costimulatory checkpoint, reducing activation of autoreactive lymphocytes and limiting the generation of new antibody-producing cells, while sparing baseline antibody responses already established. This mechanism has been explored across autoimmune conditions where T-cell-dependent B-cell activation or chronic inflammatory signaling drives tissue damage, including disorders affecting the nervous system, kidneys, skin, and connective tissue. It represents an upstream approach to immune modulation, targeting the initiating costimulatory handshake rather than downstream effector molecules.
