This mechanism uses a bispecific antibody engineered to engage two receptors from the tumor necrosis factor receptor superfamily: CD40, found on antigen-presenting cells such as dendritic cells, and 4-1BB (CD137), found on activated T cells. In normal immune physiology, CD40 engagement matures dendritic cells, prompting them to display antigen more effectively and produce signals that prime T cells, while 4-1BB engagement on those primed T cells extends their survival, proliferation, and cytotoxic activity. Combining both specificities into one molecule is intended to recreate, at the site of an immune interaction, the coordinated sequence by which a mature dendritic cell hands off an activation signal to a T cell, rather than activating either receptor in isolation throughout the body. This conditional, two-receptor design is a response to the toxicity seen when either receptor is agonized broadly and independently, since widespread activation can trigger excessive, poorly localized immune stimulation. By requiring engagement of both targets, the antibody is designed to concentrate its agonist effect where dendritic cells and T cells are physically close together, such as within a tumor or draining lymph node. This mechanism is relevant to cancers where existing immune responses are present but insufficiently potent, and where combined dendritic-cell maturation and T-cell costimulation could deepen an antitumor immune response.
