CD40 is a receptor found on antigen-presenting cells such as dendritic cells, macrophages, and B cells. It belongs to the tumor necrosis factor receptor superfamily, and its natural partner, CD40 ligand, is displayed transiently by activated T cells. Engagement of CD40 licenses antigen-presenting cells to mature: they upregulate costimulatory molecules and secrete cytokines that are required to prime robust, durable T-cell responses. This licensing step is a natural checkpoint that keeps immune activation proportionate to genuine threat. In cancer, tumors often fail to deliver adequate CD40 signaling, leaving antigen-presenting cells under-activated and T-cell priming weak. An agonist antibody directed at CD40 substitutes for the natural ligand, artificially triggering this maturation program so that antigen-presenting cells become more effective at activating tumor-reactive T cells. Because CD40 agonism must often cluster multiple receptors together to deliver a strong signal, some antibodies are engineered in their antibody-constant (Fc) region to promote this clustering through interaction with Fc receptors on nearby immune cells, rather than relying on the antibody alone. The broad rationale for this mechanism spans oncology, where converting immunologically inactive tumors into ones susceptible to T-cell attack is a central goal of combination immunotherapy strategies.
