The kappa-opioid receptor (KOR) is a G-protein-coupled receptor belonging to the opioid receptor family, alongside the mu and delta subtypes. Its natural ligand, dynorphin, activates KOR on peripheral sensory neurons and immune cells to dampen neuronal excitability and reduce the release of molecules that provoke itch and pain signaling. Because activating KOR in the brain and spinal cord can cause sedation, dysphoria, and hallucination-like effects, drug developers have designed agonists that are restricted to tissues outside the central nervous system, preserving the peripheral anti-itch and analgesic benefits of the pathway while minimizing central exposure. This peripheral-restriction strategy is particularly relevant for chronic itch conditions, such as pruritus associated with kidney disease, where the itch signal originates largely at the level of peripheral nerve endings and circulating immune mediators rather than in the brain. By engaging KOR locally, such agents can interrupt itch transmission without producing the psychoactive or sedative effects historically associated with centrally acting kappa agonists, offering a mechanistically distinct alternative to opioid mu-receptor agonists or antihistamines used for the same symptom.
