IL-1RAP (interleukin-1 receptor accessory protein) is not itself a ligand-binding receptor but a co-receptor required to complete signaling complexes for the interleukin-1 family of cytokines, most notably IL-1α and IL-1β acting through IL-1R1. Once IL-1RAP joins the ligand-bound receptor, the complex transmits a signal that activates inflammatory gene programs. Because this accessory protein is essential for signal transduction rather than ligand binding, blocking it can shut down the pathway regardless of which IL-1 family ligand is driving disease. IL-1RAP is also notable for being markedly overexpressed on the surface of certain malignant cells, including leukemic stem cells and some solid tumor cells, where IL-1 signaling can support tumor growth and suppress local immune responses. This dual biology — an inflammatory signaling hub and a tumor-associated surface marker — makes IL-1RAP a target of interest both in chronic inflammatory conditions and in oncology, where antibodies directed against it may combine cytokine pathway blockade with immune-mediated targeting of cancer cells. Monoclonal antibodies are a natural modality choice because they can engage a cell-surface protein with high specificity while also recruiting immune effector mechanisms.
