IL-12 p40 is a protein subunit shared by two related cytokines, interleukin-12 (IL-12) and interleukin-23 (IL-23). Cytokines are small signaling proteins that immune cells use to communicate. IL-12 and IL-23 each pair the p40 subunit with a distinct partner subunit, and together they direct the maturation of T helper cells into two functionally distinct populations: Th1 cells, which coordinate defense against intracellular pathogens, and Th17 cells, which are implicated in inflammation at epithelial surfaces such as skin and gut lining. In several chronic inflammatory and autoimmune diseases, these pathways become persistently overactive, driving tissue damage. Because p40 is a structural component common to both cytokines, an antibody that binds and neutralizes p40 blocks IL-12 and IL-23 signaling simultaneously, rather than targeting either pathway alone. This dual interruption addresses conditions where both Th1- and Th17-driven inflammation contribute to disease, such as certain forms of psoriasis, psoriatic arthritis, and inflammatory bowel disease. The therapeutic rationale is that suppressing this upstream shared node reduces the downstream inflammatory cascade at its source, rather than addressing individual downstream effector molecules separately. This approach exemplifies how targeting a shared building block of related cytokines can achieve broader pathway modulation than a single-cytokine-specific agent.
