The dopamine D2 and D3 receptors are G protein-coupled receptors that respond to the neurotransmitter dopamine and are concentrated in brain circuits governing movement, motivation, reward, and thought organization. Both receptor subtypes couple to inhibitory G proteins, so their activation dampens the signaling of neurons that express them. Antagonists block dopamine from engaging these receptors, which increases the net inhibitory tone on affected neurons and dampens dopaminergic signaling in the circuits involved. This is the core rationale behind most antipsychotic medicines, since excessive dopaminergic activity in certain brain pathways is strongly associated with psychosis and the positive symptoms of schizophrenia. D3 receptors are more restricted in their distribution than D2 receptors, being enriched in limbic and reward-related regions, so compounds with activity at both subtypes can influence circuits tied to reward, compulsive behavior, and mood in addition to classic psychotic symptoms. This dual D2/D3 profile broadens the therapeutic reach beyond antipsychotic effects alone, making the mechanism relevant to disorders such as schizophrenia, bipolar disorder, and certain substance-use or impulse-control conditions where dopaminergic dysregulation contributes to disease.
