The dopamine D1 receptor is a cell-surface protein belonging to the G-protein-coupled receptor family. When dopamine binds it, the receptor activates an intracellular signaling cascade—via the stimulatory G-protein Gs and the enzyme adenylyl cyclase—that raises levels of the second messenger cyclic AMP inside target neurons. D1 receptors are densely expressed in brain regions governing movement, motivation, and higher cognitive functions such as working memory and attention. Diseases marked by dopaminergic deficits, including Parkinson's disease and related disorders with Lewy body pathology, are associated with reduced dopaminergic tone and impaired D1 signaling, contributing to motor slowing and cognitive difficulties. Rather than activating the receptor directly, a positive allosteric modulator binds a site on the receptor distinct from where dopamine binds, and amplifies the receptor's response to dopamine that is already present. This approach aims to restore signaling in a manner that respects the natural timing and location of dopamine release, rather than providing constant, undifferentiated stimulation throughout the brain and body. This mechanism is of interest for conditions involving cognitive and motor impairment linked to dopaminergic circuit dysfunction, where boosting endogenous signaling, rather than replacing it outright, may offer a more physiological way of correcting deficits.
