CD19 is a transmembrane glycoprotein expressed across almost the entire B-lymphocyte lineage, from early progenitor cells through mature B cells, though it is largely absent from terminally differentiated plasma cells. Because B cells are central to antibody-mediated autoimmune disease—producing pathogenic autoantibodies, presenting antigens to T cells, and secreting inflammatory cytokines—eliminating CD19-expressing cells offers a way to interrupt these pathogenic circuits. Chimeric antigen receptor (CAR) technology can redirect immune effector cells to recognize CD19 and destroy the cells bearing it. This targeting logic, first developed for cancer using a patient's own T cells, can also be applied to autoimmune disease and paired with natural killer (NK) cells, cytotoxic lymphocytes that mediate rapid target-cell killing without requiring prior antigen sensitization. Using allogeneic NK cells sourced from healthy donors rather than autologous T cells allows for 'off-the-shelf' manufacturing, avoiding the delays associated with individualized cell collection and engineering. This mechanism broadly matters wherever pathogenic B cells sustain autoimmune disease, including conditions in which conventional immunosuppression or antibody-based B cell depletion has proven inadequate.
