Angiopoietin-2 (Ang-2) and vascular endothelial growth factor-A (VEGF-A) are two signaling proteins that together govern the growth and stability of blood vessels. VEGF-A drives new vessel formation and increases vessel permeability by acting on receptors on endothelial cells, the cells lining blood vessels. Ang-2 normally works through the Tie2 receptor to destabilize existing vessels, priming them to respond to VEGF-A and become leaky or to sprout abnormally. In diseases marked by excessive or disorganized blood vessel growth and leakage, such as neovascular eye disorders affecting the retina, both pathways become overactive and reinforce one another. A dual inhibitor that binds and neutralizes both Ang-2 and VEGF-A simultaneously addresses this reinforcing loop more completely than blocking either factor alone. This is typically achieved with a bispecific antibody, an engineered protein with two distinct binding arms, each capturing one target. By dampening both destabilizing (Ang-2) and proliferative/permeability-driving (VEGF-A) signals, this approach aims to reduce abnormal vessel leakage and growth while promoting a more stable vascular architecture. This combined strategy broadly matters in conditions where uncontrolled angiogenesis and vascular instability underlie tissue damage, particularly in ophthalmology.
