The alpha-2 adrenergic receptor is a G-protein-coupled receptor found throughout the central and peripheral nervous system, where it normally responds to the neurotransmitters norepinephrine and epinephrine. Agonists that activate this receptor, particularly in a brainstem region called the locus coeruleus, reduce the release of norepinephrine and dampen activity in circuits that maintain wakefulness and arousal. This produces a state of sedation that more closely resembles natural sleep than sedation produced by drugs acting on other systems, such as GABA-ergic agents. Because the receptor also modulates pain signaling and sympathetic nervous system tone, alpha-2 agonists are used not only for sedation but also as adjuncts in anesthesia and to blunt heart rate and blood pressure responses to stress. Selectivity for alpha-2 over the closely related alpha-1 receptor is an important design consideration, since alpha-1 activation tends to constrict blood vessels and raise blood pressure, effects generally undesirable in a sedative. Compounds designed to be relatively selective for alpha-2 receptors aim to achieve sedative and anxiolytic effects while limiting these cardiovascular effects, although at higher exposures this selectivity can narrow. This mechanism broadly matters in settings requiring controlled, reversible sedation, such as intensive care units and certain procedural or psychiatric contexts.
