Adrenergic receptors are cell-surface receptors activated by catecholamines such as epinephrine and norepinephrine, key messengers of the sympathetic nervous system. They exist as several subtypes—alpha-1, beta-1, and beta-2—each coupled to different intracellular signaling machinery and distributed unevenly across tissues. Alpha-1 receptors on blood vessel walls trigger vasoconstriction (narrowing of vessels) when activated. Beta-2 receptors, abundant in the smooth muscle surrounding the airways, trigger relaxation of that muscle when activated, widening the airway. Beta-1 receptors predominate in heart tissue, increasing heart rate and contractile force. A nonselective adrenergic agonist engages all of these subtypes at once. Delivered directly to the lungs by inhalation, this produces two complementary effects useful in acute airway obstruction: beta-2 activation relaxes constricted bronchial muscle, opening the airway, while alpha-1 activation constricts blood vessels in the airway lining, reducing the swelling and mucus congestion that also narrow the passage. Together these actions can rapidly relieve acute bronchospasm and airway congestion, relevant to conditions such as asthma. Because the agent is not selective for a single receptor subtype, it inevitably also stimulates beta-1 receptors in the heart, an effect that is not part of the therapeutic goal but cannot be avoided with a nonselective molecule.
