Airway epithelial barrier integrity refers to the physical and functional seal formed by the cells lining the respiratory tract, held together by tight junctions, adherens junctions, and a protective mucus layer. This barrier normally keeps inhaled allergens, pollutants, and pathogens from penetrating into deeper tissue where they could trigger immune activation. In chronic respiratory diseases such as asthma and chronic obstructive pulmonary disease, this barrier is often compromised: junctional proteins are downregulated, gaps appear between cells, and irritants gain easier access to underlying immune cells. This breach is thought to promote release of epithelial alarmins, molecules that alert and recruit inflammatory cells, perpetuating a cycle of inflammation and further barrier damage. An epithelial-barrier enhancer is designed to reinforce these junctional structures or support epithelial repair processes, with the therapeutic rationale being that restoring barrier function could reduce allergen and pathogen penetration, dampen downstream inflammatory signaling, and potentially lessen disease exacerbations. This mechanism is of broad interest across obstructive airway diseases and, more speculatively, in conditions where epithelial damage precedes or sustains chronic inflammation. Because it acts upstream of many immune pathways, a barrier-enhancing approach is conceptually distinct from therapies that block a single inflammatory mediator, potentially offering a more foundational point of intervention.
