Endothelial barrier function refers to the capacity of the single-cell layer lining blood vessels—the endothelium—to control what passes between blood and surrounding tissue. This barrier is maintained by specialized junctional proteins, including tight junctions and adherens junctions such as VE-cadherin, which seal adjacent endothelial cells together. When these junctions weaken, plasma fluid, proteins, and inflammatory cells leak into tissue, producing edema (swelling) and, in severe cases, organ dysfunction. This process, often called endothelial dysfunction or vascular leak, is a central feature of conditions such as sepsis, acute respiratory distress syndrome, diabetic macular edema, and certain inflammatory or ischemic disorders. Agents classified as endothelial dysfunction blockers aim to reinforce or restore junctional integrity, reducing pathological permeability without abolishing the endothelium's normal, tightly regulated exchange functions. The rationale for targeting this mechanism broadly, rather than a single upstream trigger, is that many distinct insults—infection, hypoxia, inflammatory cytokines, hyperglycemia—converge on the same final common pathway of junctional breakdown. Stabilizing the barrier directly may therefore offer benefit across multiple disease contexts that share vascular leak as a downstream consequence, independent of the specific inciting cause. This makes endothelial barrier stabilization a mechanism of interest wherever excessive vascular permeability contributes meaningfully to tissue injury or organ failure.
