Randomized, double-blind phase 2 in relapsing-remitting multiple sclerosis (220 participants). Both ocrelizumab doses reduced gadolinium-enhancing brain lesions decisively versus placebo (p<0.0001); the endpoint is an MRI biomarker rather than a clinical relapse or disability outcome, which is why the grade is moderate despite the clear result.

This was a large, randomized, quadruple-blind, active-controlled trial of 955 participants, comparing ofatumumab with teriflunomide. The prespecified primary clinical endpoint, annualized confirmed relapse rate, was significantly better with ofatumumab (p<0.001), supporting a strong evidence grade despite mixed findings for some secondary measures.
