TL1A (TNF-like ligand 1A) is a signaling protein belonging to the tumor necrosis factor (TNF) superfamily. It is produced by immune cells such as macrophages, dendritic cells, and endothelial cells, particularly at sites of tissue inflammation. TL1A acts on a receptor called DR3 (death receptor 3), found on T cells and other immune cells, amplifying their activation and promoting production of inflammatory mediators. This TL1A–DR3 axis has been implicated in chronic inflammatory and fibrotic conditions, most notably inflammatory bowel diseases such as ulcerative colitis and Crohn's disease, where sustained immune activation damages the intestinal lining and can lead to scarring (fibrosis) over time. Blocking TL1A with a monoclonal antibody is intended to dampen this upstream inflammatory signal before it triggers the broader cascade of immune cell recruitment and tissue damage. Because TL1A sits near the start of this pathway, its inhibition is thought to reduce both active inflammation and the fibrotic remodeling that can follow it, distinguishing it from therapies that act further downstream. Some agents in development pair anti-TL1A activity with blockade of a second inflammatory pathway, reflecting the view that inflammatory bowel disease is driven by more than one contributing signal.
