TIM receptors form a family of transmembrane proteins on immune cells, distinguished by immunoglobulin and mucin-like extracellular domains that recognize phosphatidylserine, a lipid marker exposed on the surface of dying cells. Beyond helping clear apoptotic debris, members of this family shape T cell activation, exhaustion, and differentiation into regulatory T cells (Tregs), a subset that restrains other immune cells and maintains tolerance to the body's own tissues. An agonist designed to engage a TIM receptor and expand Tregs pursues immune rebalancing rather than blanket suppression, a strategy relevant to autoimmune and chronic inflammatory diseases such as lupus, inflammatory bowel disease, or multiple sclerosis, where autoreactive or inflammatory effector T cells outpace the regulatory population meant to contain them. By promoting expansion or stability of Tregs rather than depleting or blocking effector T cells outright, this mechanism aims to restore a more physiological immune balance while limiting the broad immunosuppression associated with global T-cell-targeting therapies, which raises infection risk. Delivering this activity through an oral small molecule, rather than an injected antibody, is pharmacologically demanding because achieving selective, potent engagement of a receptor within this closely related family without off-target immune effects is technically difficult.
