TIM-3 (T-cell immunoglobulin and mucin domain-containing protein 3) is a cell-surface receptor found on T lymphocytes, natural killer cells, and certain myeloid cells. It belongs to a family of inhibitory receptors, sometimes called immune checkpoints, that dampen immune activity once cells engage their ligands. In chronic settings such as persistent infection or cancer, T cells that are repeatedly stimulated by antigen become progressively less functional, a state termed exhaustion, and TIM-3 is one of several receptors that accumulate on these exhausted cells alongside others like PD-1. Blocking TIM-3 with an inhibitor is intended to interrupt this inhibitory signaling, allowing exhausted immune cells to regain some capacity to recognize and attack tumor cells or infected cells. Because TIM-3 is frequently co-expressed with other checkpoint receptors on the most exhausted T cell subsets, it is often considered a marker of advanced dysfunction and a potential complement to blocking earlier checkpoints such as PD-1. The broad rationale for targeting TIM-3 spans oncology, where restoring T cell and natural killer cell activity against tumors is desired, and has also been explored in chronic viral infection, where similar exhaustion phenomena limit pathogen clearance. As with other checkpoint-directed approaches, the underlying principle is to remove a brake on immune surveillance rather than to directly kill target cells.
