Renin is an aspartyl protease produced primarily by kidney cells that catalyzes the first, rate-limiting step of the renin-angiotensin-aldosterone system (RAAS): cleavage of angiotensinogen to angiotensin I, which is then converted to angiotensin II, a potent vasoconstrictor and driver of aldosterone release. Inhibiting renin blocks this cascade at its origin, and systemic renin inhibitors have been explored as a way to lower blood pressure by limiting angiotensin II generation. Beyond the classical circulating system, local RAAS components are expressed in many tissues, including the intestinal wall, where locally generated angiotensin II can contribute to inflammation, fibrosis, and altered vascular tone independent of the circulating hormone pool. A gut-restricted renin inhibitor is designed to act only within the intestinal compartment, inhibiting local renin activity without being absorbed into the bloodstream in appreciable amounts. This selectivity aims to modulate tissue-level RAAS-driven pathology, such as intestinal inflammation or fibrotic remodeling, while avoiding the systemic blood pressure lowering, electrolyte disturbances, and other consequences of whole-body renin inhibition. This approach matters wherever local RAAS activation drives tissue injury, offering compartment-specific intervention rather than systemic hormonal suppression.
