This mechanism refers to a live biotherapeutic product: an orally administered, multi-strain formulation of selected bacteria intended to metabolize waste compounds that accumulate in the blood when kidney function declines. In chronic kidney disease, the kidneys lose their capacity to filter nitrogenous and other metabolic byproducts, including urea, uric acid, and creatinine, collectively termed uremic toxins. Because these small molecules diffuse across the gut lining along a concentration gradient, the intestine functions as a secondary route of exchange between blood and gut contents. The biological rationale, sometimes called "enteric dialysis," is to introduce or expand bacterial populations in the colon that consume these toxins as they diffuse in, using them as nutrients and thereby lowering the concentration gradient and enhancing continued diffusion, and metabolizing them into products excreted in stool rather than reabsorbed. Using multiple bacterial strains, rather than a single organism, broadens the range of toxins addressed and improves the likelihood that at least some strains establish and function reliably despite variation in individual gut environments. This approach is relevant chiefly in earlier and moderate stages of chronic kidney disease, before dialysis or transplantation becomes necessary, as a way to reduce toxin burden and potentially slow the accumulation of uremic symptoms.
