The P2Y12 receptor is a cell-surface receptor found on platelets, the small blood cells responsible for forming clots. It is activated by adenosine diphosphate (ADP), a molecule released from activated platelets and damaged tissue, which reinforces platelet activation and aggregation once an initial clotting signal has occurred. This amplification step is central to thrombus formation, particularly in arteries where blood flow is fast and clots can occlude vessels supplying the heart or brain. Blocking the P2Y12 receptor interrupts this feedback loop, reducing the ability of platelets to aggregate and stabilize a clot without eliminating platelet function entirely, since other activation pathways remain intact. This selective dampening is the rationale for using P2Y12 antagonists in conditions where excessive clotting poses acute risk, such as during and after procedures that involve blood vessel injury or stenting, and in the management of acute coronary syndromes. Because platelets play an essential protective role in stopping bleeding after injury, modulating this receptor requires a balance between reducing pathological clot formation and preserving enough platelet reactivity to prevent excessive bleeding. This trade-off underlies much of the clinical reasoning around antiplatelet therapy targeting this receptor.
