OX40 (CD134) is a costimulatory receptor belonging to the tumor necrosis factor receptor superfamily, expressed mainly on the surface of recently activated T lymphocytes. Its natural partner, OX40 ligand (OX40L), is displayed by antigen-presenting cells such as dendritic cells during an immune response. Engagement of OX40 delivers a secondary signal that reinforces the primary T-cell receptor signal, extending T-cell survival, promoting proliferation, and enhancing the release of effector cytokines. Because this receptor amplifies rather than initiates immune activation, it is an attractive target for therapies designed to strengthen an existing but insufficient immune response. In oncology, agonist antibodies directed at OX40 are explored as a way to sustain the activity of tumor-infiltrating T cells, which might otherwise become exhausted or die prematurely within the suppressive tumor microenvironment. This mechanism sits within the broader family of immune-checkpoint and costimulatory approaches that aim to convert a weak antitumor immune response into a durable one. Because OX40 also affects regulatory T cells, its modulation intersects with the balance between promoting effective immunity and controlling autoimmunity, making this pathway relevant across oncology and, in other contexts, immune-mediated inflammatory disease.
