Norepinephrine is a signaling molecule (a catecholamine) released by sympathetic nerves that constricts blood vessels and helps maintain blood pressure, especially when a person stands upright. In some neurological conditions, the autonomic nervous system fails to release enough norepinephrine, causing blood pressure to fall sharply on standing (neurogenic orthostatic hypotension), leading to dizziness or fainting. Rather than directly activating the receptors that norepinephrine acts on, one therapeutic strategy is to supply the body with a precursor molecule that it can convert into norepinephrine itself. This precursor is absorbed and processed by an enzyme naturally present in nerve tissue and other organs, raising circulating norepinephrine levels and helping restore vascular tone. This approach is used where the deficiency lies in norepinephrine production or release rather than in the receptors that respond to it, making receptor-targeted drugs less suitable. The broader relevance of this mechanism spans disorders of autonomic failure, including those associated with certain neurodegenerative diseases and peripheral neuropathies, where insufficient norepinephrine release undermines cardiovascular regulation upon postural change.
