Serotonin (5-HT) and norepinephrine (NE) are chemical messengers, called monoamines, released by neurons that project from small brainstem centers—the raphe nuclei and the locus coeruleus—into wide areas of the cerebral cortex. There they help regulate arousal, attention, mood, and the planning of movement. A "cortical enhancer" acting on both systems aims to increase the activity or availability of these two neurotransmitters specifically within cortical circuits, strengthening signaling that supports concentration, working memory, and coordinated motor output. The rationale for enhancing both 5-HT and NE together, rather than one alone, is that each acts through different receptor families and contributes distinct functions in the prefrontal cortex: norepinephrine sharpens the precision of neural signaling underlying attention and working memory, while serotonin contributes to mood stability and impulse control. Combining the two can address deficits that neither system alone fully explains. This general mechanism is relevant across conditions involving cortical under-activity, including attention disorders, certain mood disorders, and neurodegenerative diseases where loss of monoaminergic input to the frontal cortex contributes to impaired executive function, attention, and postural or gait control.
