MRGPRX2 (Mas-related G protein-coupled receptor member X2) is a cell-surface receptor found predominantly on mast cells, immune cells that store histamine, proteases, and inflammatory cytokines in granules. Unlike the classical IgE receptor that triggers mast cells during allergic reactions, MRGPRX2 responds to a distinct set of triggers, including neuropeptides such as substance P and PACAP that are released from sensory nerve endings in the skin. When these neuropeptides engage MRGPRX2, mast cells degranulate, releasing mediators that cause itch, redness, and swelling, and that further sensitize the same sensory neurons, creating a self-reinforcing neuroimmune loop. This pathway is thought to underlie itch and inflammation in conditions such as chronic urticaria, atopic dermatitis, and other pruritic or neurogenic inflammatory skin disorders that respond poorly to antihistamines because they are not driven by IgE. An MRGPRX2 antagonist is designed to interrupt this receptor's signaling, dampening mast-cell activation at its source without disturbing IgE-mediated allergic responses, offering a mechanistically distinct approach for itch and inflammation that persist despite conventional antiallergic therapy.
