Lipoprotein(a), or Lp(a), is a cholesterol-carrying particle in blood that resembles low-density lipoprotein (LDL) but carries an additional protein called apolipoprotein(a) attached to it. Elevated blood levels of Lp(a) are strongly and independently associated with atherosclerotic cardiovascular disease and with calcific aortic valve disease. Unlike LDL cholesterol, Lp(a) concentration is determined almost entirely by genetics, is present from birth, and is not meaningfully lowered by diet, exercise, or standard cholesterol-lowering drugs such as statins. This has created interest in therapies that act directly on the biological machinery producing the particle. Antisense oligonucleotides targeting apolipoprotein(a) work by binding the messenger RNA that codes for this protein inside liver cells, marking it for degradation before it can be translated into protein. Because apolipoprotein(a) is required for the assembly and secretion of a complete Lp(a) particle, reducing its production lowers the number of particles released into circulation. This approach is relevant to any condition where persistently elevated Lp(a) is thought to contribute to vascular or valvular disease risk, particularly in individuals whose risk is not fully explained by other measured lipid abnormalities.
