Ketone-body metabolism refers to the biochemical pathway by which the liver converts fatty acids into small energy-carrying molecules called ketone bodies—principally acetoacetate and beta-hydroxybutyrate—during periods of low carbohydrate availability, such as fasting or a very low-carbohydrate diet. These molecules travel through the bloodstream and are taken up by tissues, including the brain, where they serve as an alternative fuel to glucose. Modulating this pathway therapeutically means deliberately raising circulating ketone levels to supply energy to tissues that have difficulty using glucose efficiently. This strategy is relevant in neurological conditions marked by impaired glucose transport or utilization in the brain, including certain epileptic syndromes and neurodegenerative disease, where ketone bodies can be metabolized through a separate route and may help sustain cellular energy production. Medium-chain triglycerides, a class of dietary fats with shorter fatty-acid chains than typical dietary fats, are used as a practical tool to induce this state because they are metabolized rapidly and preferentially into ketone bodies without requiring the same degree of dietary carbohydrate restriction as a classic ketogenic diet. This makes the approach a metabolic, rather than a molecularly targeted, therapy.
