IL-6 is a cytokine that coordinates inflammation, immune defense, and tissue repair. It signals through two routes. In classic signaling, IL-6 binds a membrane-bound receptor present mainly on hepatocytes and certain immune cells, triggering protective responses such as the acute-phase reaction and immune regulation. In trans-signaling, IL-6 first binds a soluble form of its receptor circulating in blood, and this complex can then activate the signaling partner protein gp130 on virtually any cell, including endothelial cells, fibroblasts, and epithelial cells that lack the membrane receptor. This broadens IL-6's reach and is disproportionately responsible for chronic, tissue-damaging inflammation seen in conditions such as inflammatory bowel disease, rheumatoid arthritis, and certain fibrotic and vascular disorders. A selective trans-signaling inhibitor is designed to neutralize only the IL-6/soluble-receptor complex, leaving classic signaling intact. The rationale is to dampen pathological inflammation while preserving IL-6's homeostatic roles in host defense, liver regeneration, and metabolic regulation, potentially offering a more selective safety profile than agents that block IL-6 signaling altogether.
