Interleukin-31 receptor A (IL-31RA) is one half of a two-part receptor complex, pairing with oncostatin M receptor beta (OSMRβ), that senses the cytokine IL-31. IL-31 is released mainly by type 2 helper T cells and mast cells during allergic and inflammatory skin responses. When it engages IL-31RA/OSMRβ on sensory nerve endings, keratinocytes, and immune cells, it triggers intracellular signaling that produces itch sensation and reinforces skin inflammation and barrier disruption. Because chronic itch is a defining, disabling feature of conditions such as atopic dermatitis and prurigo nodularis, and conventional anti-inflammatory therapies often control skin lesions without adequately relieving itch, IL-31RA has become a rational target in its own right. Antibodies directed against IL-31RA occupy the receptor and prevent IL-31 from binding, interrupting both the neural itch signal and the cytokine's contribution to inflammatory skin changes. This dual action—addressing sensation and underlying inflammation—distinguishes IL-31RA blockade from therapies that target only classic inflammatory cytokines. The approach is broadly relevant across pruritic skin diseases where itch and inflammation are interlinked, offering a route to symptom relief that is more targeted than systemic immunosuppression.
