Interleukin-15 (IL-15) is a cytokine, a small signaling protein, that supports the development, survival, and activation of natural killer (NK) cells and certain populations of T lymphocytes, particularly memory CD8+ T cells and specialized cells residing in tissues such as the gut lining and skin. It signals through a receptor complex that is presented in a distinctive manner: rather than simply binding a free receptor on the responding cell, IL-15 is often held by a presenting cell and handed across to the immune cell, a process called trans-presentation. This tight, localized signaling makes IL-15 a driver of chronic inflammation in certain autoimmune and inflammatory diseases, where excessive IL-15 activity sustains abnormal T-cell and NK-cell responses against the body's own tissues. An anti-IL-15 monoclonal antibody works by binding circulating or cell-surface IL-15 and preventing it from engaging its receptor complex, thereby dampening the downstream activation of these lymphocyte populations. This mechanism is of broad interest in conditions where tissue-damaging immune activity is linked to IL-15 overactivity, including certain autoimmune enteropathies and inflammatory skin and mucosal disorders. The general therapeutic rationale is to interrupt a specific inflammatory signal without broadly suppressing the entire immune system.
