The gut-brain axis refers to the bidirectional communication network linking the gastrointestinal tract and the central nervous system. This communication occurs through several parallel routes: the vagus nerve, which carries sensory signals from the gut to the brainstem; circulating immune mediators such as cytokines; and metabolites produced by the resident gut microbiota, including short-chain fatty acids and neurotransmitter precursors. Because gut microbial communities influence all of these signaling routes, altering the composition or metabolic activity of that microbiota is one strategy for influencing central nervous system function indirectly, without a drug needing to cross the blood-brain barrier itself.

Live biotherapeutics are formulations of living microorganisms, often selected or engineered bacterial strains, administered as a treatment to recolonize or modulate the gut microbial ecosystem. Lactococcus lactis is a lactic acid bacterium commonly used as a delivery vehicle in this context because it is well characterized and can be adapted to produce specific molecules locally in the gut.

This mechanism is broadly relevant to conditions where gut-brain signaling is implicated, including functional gastrointestinal disorders, mood and anxiety-related symptoms associated with gastrointestinal disease, and other conditions where inflammation or microbial imbalance in the gut is thought to influence central nervous system function.
