Mechanism

Dopamine D2 and serotonin 5-HT1A receptors (partial agonist); serotonin 5-HT2A receptor (antagonist)

Assets acting on this target.

Class
Atypical (third-generation) antipsychotic; dopamine-serotonin system stabilizer; the aripiprazole tablet (same molecule as Abilify) is embedded with an ingestible Proteus sensor as part of the Abilify MyCite Digital Medicine System, which pairs with a wearable patch and app to track adherence
Pathway
Partial agonism at D2/5-HT1A combined with 5-HT2A antagonism produces a dopamine-serotonin stabilizing effect thought to underlie efficacy in schizophrenia, bipolar I disorder, and adjunctive MDD treatment with lower extrapyramidal symptom risk than full D2 antagonists

This mechanism involves three receptors in brain dopamine and serotonin signaling systems, both implicated in psychotic and mood disorders. The molecule acts as a partial agonist at dopamine D2 receptors, meaning it activates the receptor but less strongly than the natural neurotransmitter dopamine itself. In brain regions where dopamine signaling is excessive, this partial activity effectively dampens transmission; in regions where dopamine tone is low, the same partial agonism provides a degree of baseline stimulation. This bidirectional action is described as dopamine system stabilization, distinguishing it from older antipsychotics that fully block D2 receptors everywhere. The compound also partially activates serotonin 5-HT1A receptors and blocks 5-HT2A receptors, actions that further modulate dopamine release in specific pathways and contribute to effects on mood and cognition. Together, these three receptor actions are thought to reduce psychotic symptoms while lowering the risk of movement-related side effects (extrapyramidal symptoms) and excessive prolactin elevation that are more common with full D2 antagonists. This receptor profile broadly matters in schizophrenia, bipolar disorder, and as an add-on approach in depression that has not adequately responded to standard antidepressant treatment, conditions in which dopamine and serotonin circuit imbalances are believed to contribute to symptoms.
