CSF-1R inhibitor (microglial modulator)

CSF-1R (colony-stimulating factor 1 receptor, also called c-Fms) is a cell-surface receptor tyrosine kinase found mainly on cells of the monocyte-macrophage lineage, including microglia, the resident immune cells of the brain. It is activated by two natural ligands, CSF-1 and IL-34, which drive the differentiation, proliferation, and survival of these myeloid cells. Because macrophages and microglia can sustain harmful inflammation or support tumor growth by remodeling their surrounding tissue, blocking CSF-1R signaling is a strategy to reduce the number or activity of these cells. In oncology, this can limit tumor-associated macrophages that otherwise shield cancer cells from immune attack. In neurology, dampening microglial activation is being explored for neurodegenerative and neuroinflammatory conditions where overactive microglia contribute to tissue damage. In certain benign tumors driven by CSF-1 overexpression, CSF-1R inhibition can directly reduce the abnormal proliferation of macrophage-like cells within the tumor mass. Small-molecule inhibitors, such as the one referenced here, block the kinase activity of the receptor, preventing downstream signal transduction regardless of which ligand engages it. This class of mechanism sits at the intersection of oncology and neurology because the same myeloid cell biology underlies tumor microenvironment support and central nervous system inflammation.
