CD52 is a small protein anchored to the outer surface of mature lymphocytes (T cells and B cells), with lower-level expression on monocytes and some other immune cells. Its precise physiological role is not fully defined, but its abundance and even distribution across the lymphocyte surface make it an efficient target for antibody binding. An antibody directed against CD52 marks these cells for destruction by the body's own immune effector systems, producing a broad and rapid depletion of circulating lymphocytes. This strategy is used where disease is driven by populations of lymphocytes that are behaving inappropriately, either malignant lymphocytes proliferating uncontrollably or autoreactive lymphocytes attacking the body's own tissues. By clearing out much of the circulating lymphocyte pool, the underlying disease process can be interrupted, and in autoimmune contexts the subsequent slow regeneration of the immune system may result in a more balanced, less self-reactive repertoire. This class of therapy is relevant to certain blood cancers and to autoimmune neurological disease, wherever suppressing or resetting adaptive immunity is therapeutically useful.
