Allogeneic bone-marrow-derived mesenchymal stem cell therapy

Mesenchymal stem cells (MSCs) are a type of adult stromal cell, originally isolated from bone marrow but also obtainable from tissues such as umbilical cord blood, that can be expanded in the laboratory and administered to a recipient different from the donor (an allogeneic approach). Rather than replacing damaged tissue by turning into new cells at the site of injury, their principal therapeutic action is thought to be immunomodulatory: they release signaling molecules that dampen excessive activation of T cells, natural killer cells, and inflammatory macrophages, while promoting a more regulatory, tissue-protective immune environment. This property makes them of interest in conditions driven by dysregulated or overactive immune responses, most notably graft-versus-host disease following bone marrow or stem cell transplantation, as well as a range of inflammatory and degenerative conditions where excessive immune activity contributes to tissue damage. Using allogeneic, donor-derived cells rather than a patient's own cells allows product to be manufactured in advance from a single donor source and used across many recipients, which addresses a practical need for rapid availability in acute inflammatory settings where there is no time to harvest and expand a patient's own cells. This mechanism broadly matters wherever immune overactivation, rather than an absence of tissue, is the primary driver of disease.
