Activin receptor type IIA (ActRIIA) is a cell-surface receptor belonging to the transforming growth factor-beta (TGF-β) superfamily of signaling receptors. Its natural ligands include activins, myostatin-related factors, and growth differentiation factors (GDFs), which bind ActRIIA and its co-receptors to activate intracellular SMAD2/3 signaling, a pathway that influences cell growth, tissue remodeling, and vascular smooth muscle proliferation. In several diseases, excessive activin/GDF signaling through this pathway drives pathological tissue changes: in the pulmonary vasculature, it contributes to thickening and remodeling of small arteries, a hallmark of pulmonary arterial hypertension. Modulating ActRIIA signaling is therefore pursued as a way to rebalance growth-promoting and growth-restraining signals in affected tissues. One common therapeutic strategy is a 'ligand trap' - typically a fusion protein combining the extracellular ligand-binding domain of ActRIIA with an antibody Fc region - that sequesters circulating ligands before they can engage the native receptor, effectively reducing pathway activity without directly blocking the receptor itself. This approach is being explored not only in pulmonary vascular disease but also in other conditions where activin-family signaling promotes abnormal tissue growth, including certain hematologic and musculoskeletal disorders.
