Head-to-head evidence

Dapagliflozin vs Empagliflozin

Same mechanism, same race: design-graded trial evidence for both assets, with the grading rationale shown. Grades describe the quality of the evidence, never the merits of either security.

Type 2 Diabetes

Shared mechanism: SGLT2 · full Type 2 Diabetes pipeline

Dapagliflozin · Farxiga / Forxiga

Marketed by AstraZeneca

Approved in Type 2 Diabetessmall molecule
Partial signal

NCT00528372 · Phase 3 · completed

The study was randomized, double-blind, placebo-controlled, and included 1,067 enrolled participants, with 558 receiving short-term treatment; its primary HbA1c endpoint is an accepted disease-defining measure for type 2 diabetes. Results were mixed across the primary dose comparisons: 5 mg and 10 mg morning doses significantly outperformed placebo, while 2.5 mg did not, and the exploratory high-baseline-HbA1c group lacked a comparator.

Marketed by Boehringer Ingelheim and partners

Approved in Type 2 Diabetessmall molecule
Reliable signal

NCT01131676 · Phase 3 · completed

This was a randomized, double-blind, placebo-controlled trial enrolling 7,064 participants. The hard clinical primary endpoint was met: pooled empagliflozin was non-inferior and also statistically superior to placebo for 3-point MACE, with 10.5% versus 12.1% of participants experiencing an event (HR 0.86; p=0.0382).

Reliable signal

NCT03036150 · Phase 3 · completed

This was a large, randomized, double-blind, placebo-controlled trial measuring a hard clinical composite of kidney failure and death, and dapagliflozin significantly reduced this composite compared with placebo.

Approved in Chronic Kidney Diseasesmall molecule
Reliable signal

NCT03594110 · Phase 3 · completed

Patients were randomized to empagliflozin or matching placebo under double-blind conditions in a large trial, with a hard clinical composite endpoint covering kidney failure, substantial kidney function decline, kidney-related death, or cardiovascular death. Empagliflozin significantly reduced this outcome compared with placebo (hazard ratio 0.72, 95% CI 0.59-0.89, p<0.0001).

Marketed by Boehringer Ingelheim and partners

Approved in Heart Failuresmall molecule
Reliable signal

NCT03057951 · Phase 3 · completed

This trial randomized patients to empagliflozin or matching placebo under double-blind conditions in a very large group, and measured a hard clinical outcome: cardiovascular death or hospitalization for heart failure. Empagliflozin significantly lowered this combined event rate compared with placebo (hazard ratio 0.79, 95% CI 0.69-0.90, p=0.0003).
