TROP2 and Nectin-4 are two proteins found on the surface of epithelial cells that are frequently overexpressed in cancers arising from lung, breast, and bladder tissue. Each protein has an independent biological role—TROP2 participates in calcium-linked growth signaling, while Nectin-4 mediates cell-to-cell adhesion—but both are exploited in oncology because tumor cells display them at markedly higher density than most normal tissues. This mechanism uses a bispecific antibody engineered to recognize both antigens at once, linked to a cytotoxic payload, forming what is termed an antibody-drug conjugate (ADC). The rationale for engaging two targets rather than one is to increase the specificity with which the antibody component identifies malignant cells: tumors that co-express TROP2 and Nectin-4 provide a more distinctive molecular signature than either antigen alone, which can improve tumor cell recognition and payload delivery while potentially sparing normal tissue that expresses only one of the two proteins. Once bound and internalized, the ADC releases its cytotoxic drug intracellularly, killing the cell from within rather than relying solely on antibody-mediated immune effects. This dual-antigen strategy is broadly relevant to solid tumors of epithelial origin, where antigen heterogeneity within and between tumors is a recognized obstacle to single-target therapies.
