TrkA, also called NTRK1, is a receptor tyrosine kinase and the principal high-affinity receptor for nerve growth factor (NGF), a signaling protein that regulates the growth, survival, and function of sensory neurons. When NGF binds TrkA on peripheral nerve endings, the receptor dimerizes and autophosphorylates, triggering intracellular signaling cascades that sensitize nociceptors—the nerve terminals that detect pain and itch stimuli. This sensitization lowers the threshold for pain signaling and contributes to chronic pain and pruritus in conditions such as osteoarthritis, chronic itch disorders, and inflammatory states where local NGF levels rise in affected tissue. Small molecules that inhibit TrkA kinase activity block this signaling cascade, blunting peripheral sensitization at its source. Formulating such an inhibitor topically is a deliberate strategy: applying it directly to skin concentrates drug exposure locally, limiting systemic absorption and reducing the chance of disrupting NGF-TrkA signaling elsewhere in the body, where this pathway supports neuronal maintenance and other physiological roles. This localized approach aims to achieve a therapeutic effect on pain or itch in the treated area while minimizing the broader systemic pharmacology associated with whole-body NGF pathway blockade.
