TREM-1 (Triggering Receptor Expressed on Myeloid cells-1) is a cell-surface receptor found mainly on neutrophils and monocytes/macrophages, the immune cells that mount the body's first response to infection or tissue injury. TREM-1 does not sense pathogens directly; instead, it acts as an amplifier of signals coming from pattern-recognition receptors such as Toll-like receptors, magnifying the release of inflammatory mediators once an infection or injury is detected. This amplification is useful in moderation, but in severe infection or critical illness it can become excessive, contributing to the runaway, tissue-damaging inflammation seen in conditions such as sepsis and septic shock, as well as some acute organ injuries and chronic inflammatory diseases. Blocking TREM-1 with an antagonist aims to dial back this amplification loop, reducing the exaggerated inflammatory output while leaving the core pathogen-sensing machinery intact, so that the immune system can still recognize and respond to infection without swinging into the harmful hyperinflammatory extreme. This selective dampening strategy is the biological rationale for developing TREM-1-directed therapies, particularly in acute inflammatory syndromes where inflammation itself, rather than the infection alone, drives organ damage and mortality.
