A therapeutic cancer vaccine is designed to teach the immune system to recognize and attack existing tumor cells, in contrast to a preventive vaccine that blocks infection before disease occurs. Peptide-based vaccines use short protein fragments derived from proteins that are overexpressed or aberrantly expressed on cancer cells (tumor-associated antigens) or that arise from mutations unique to the tumor (neoantigens). When injected under the skin, these peptides travel to nearby lymph nodes, where specialized immune cells called antigen-presenting cells take them up and display them on major histocompatibility complex (MHC) molecules. This display activates T lymphocytes, the immune cells responsible for killing infected or abnormal cells, training them to recognize and destroy tumor cells bearing the same antigen. The broad rationale for this approach is that tumors often evade natural immune surveillance by resembling normal tissue closely enough to avoid detection; vaccination aims to sharpen that recognition. This mechanism is relevant across many solid tumor types, particularly where a well-defined, tumor-restricted antigen exists, and it is frequently explored in combination with other immune-modulating therapies to strengthen or sustain the resulting T-cell response.
