ROR1 (receptor tyrosine kinase-like orphan receptor 1) is a cell-surface protein normally expressed during embryonic development and largely silenced in adult tissues, but it reappears on the surface of several blood cancers and solid tumors, making it an attractive tumor-selective marker. Rather than blocking ROR1 signaling directly, this mechanism uses it as a docking site to redirect the immune system. A tetraspecific antibody is engineered with multiple binding arms: one anchors to ROR1 on the tumor cell, another engages CD3 on a T cell to trigger its cytotoxic program, and additional arms engage a costimulatory receptor on the same T cell, reinforcing the activation signal. This design reflects a broader principle in T-cell-engaging therapy: T cells require more than one signal to become fully and durably activated, and providing that second signal alongside the first can improve killing of tumor cells while potentially reducing excessive, non-specific immune activation. Diseases where this approach broadly matters include hematologic malignancies and certain solid tumors that re-express oncofetal antigens like ROR1, where conventional therapies may fail to distinguish tumor from healthy tissue. The overall rationale is to harness the precision of the adaptive immune system, directing cytotoxic T cells specifically toward cells bearing an antigen that is largely absent from normal adult tissue.
